*As the weather warms up, please remember to apply sunscreen and bug spray prior to arriving to school! 

*Any child requiring medication during the school day (EpiPen, EpiPen Jr.,inhalers, nebulizer solution, daily medications) must have a completed Medication Administration form with a physician's signature on file.  Medication MUST be provided by the family in the pharmacy labeled container. Allergy and Asthma Action plans are also available for download. 

*Medications that will be used for a short course (ten days or less) do not require a Medication Administration form signed by the physician. 

 

  
Health News

If your child is ill (or sent home from school ill), please refrain from sending him/her back to school using the following guidleines for common communicable diseases:

Vomiting-Keep your child home until he/she is symptom free and well hydrated for a minimum of 24 hours.

Conjunctivitus/Pink Eye-Keep your child home until he/she has been on medication for 24 hours.

Diarrhea-Keep your child home until he/she is symptom free and well hydrated for 24 hours.

Fever and Flu-Keep your child home until he/she is symtpom free for 24 hours. If your child has a fever in the evening, but is without a fever in the morning, please refrain from sending them to school.

Lice-Keep your child home until he/she has been treated and live lice and nits are removed.

Scabies-Keep your child home until he/she has been treated.

Strep Throat/Scarlet Fever-Keep your child home until he/she is fever free and has been on medication for 24 hours.

Physical Examinations- Documentation of a current physical examination (within 12 months prior to first day of school entry and at intervals of every three to four years thereafter) must be on file in the health office. Any student wanting to participate in sports must have a yearly physical examination on file in the health office prior to the start of the season. Please check with the school nurse to make sure your child is in compliance.

Announcements

Documentation of the listed vaccines is required by the Department of Public Health. These must be on file in the health office prior to the start of the 2012-2013 school year unless there is religious or medical exemption.  Updated information may be faxed to (508) 278-0391. Any claim of religious or medical exemption must be submitted in writing to the health office.  

By Age Two

3 doses of Hep B, 4 doses of DTaP/DPT,  3 doses of Polio,  3 or more doses of Hib,  1 dose of MMR,  1 dose of Varicella

By Kindergarten                       

3 doses of Hep B,  5 doses of DTaP/DTP,  4 doses of Polio,  2 doses MMR,  2 doses Varicella 

By 7th Grade   

 3 doses Hep B, 1 booster dose of Td or Tdap, >3 doses Polio, 2 doses MMR, 2 doses Varicella
